Penile Implant Surgery in India
Penile implant surgery is a process that helps them with erectile dysfunction disease occurring in men. Apart from erectile dysfunction there is no accountability in any other treatment. In penile implant surgery, a prosthetic device is inserted inside the penis and testicles. This device helps men get an erection and perform a sexual function. It is a medical device that is transplanted during the penis corpora cavernosa during surgery. This device is indicated for use in men with treatment-resistant impotence or erectile dysfunction.
Who can undergo a penile implant surgery?
Erectile dysfunction (ED), or impotence, is one of the most common penile disorders that affect men. In this, the patient is not able to achieve or sustain an erection in the penis. This may prevent conception and intercourse. A penile implant surgery is advised in certain conditions for men suffering from ED.
There are various reasons for a person suffering from erectile dysfunction. It could be the result of anxiety, depression, medications or drugs. In such circumstances, the condition can be treated easily and an implant will not be advised.
How is a Penile Implant surgery performed?
The patient gets to choose the type of device to be used once the decision to undergo the surgery has been taken.
Mainly, there are 3 types of penile implants:
- Rods
- Inflatable Devices
- Complex Inflatable Devices
A rod implant is one of the most common and affordable types of penile implants. It can be easily implanted and gives a permanent erection. However, the abnormal appearance of the penis may one of the disadvantages of this type of penile implant.
Simple inflatable devices can be used to give erection at will. It has an easy-to-use inflating and deflating pump to help achieve and lose an erection. This is a comparatively more complex and an expensive procedure. One of the disadvantages of this device is that it may not give as much erection as a rod penile implant and may not deflate completely.
Complex inflatable devices are considered to be the best devices for treating erectile dysfunction. However the procedure to implant these is comparatively more complicated than others and the procedure is also more expensive. Risk of complications from this surgery is also very high. The most complex type of this device is composed of three parts, where one is placed in each of three genital parts – the penis, scrotum and the abdomen. This device is highly concealed and gives a natural looking erection, with a larger-sized penis than before. The system of inflating and deflating the device works efficiently to achieve and lose erections.
The exact surgical procedure mainly depends on the type of implant being used. For a rod and simple inflatable device implant, the surgery may be done as an outpatient procedure. However, for implanting complex inflatable device, the surgery is longer and the patient may be required to stay in the hospital for a few days following the surgery.
General anesthesia is usually administered before the implant surgery to prevent pain or discomfort to the patient during the surgery. The incision for implanting the device may be done on the penis, scrotum or the on the skin above or below the penis and scrotum. The device and the individual surgeon’s preferred method will dictate the types and locations of the incisions.
Inflatable implant devices are generally made of a saline reservoir, a pump which is implanted in the scrotum and two tube-like cylinders which are implanted in the penile shaft. As the saline solution is pumped into the penis, it causes an erection. The pump also helps to remove the saline solution and lose the erection.
The non-inflatable penile implant devices normally consist of a set of dual rods that are semi-rigid yet flexible. These are implanted within the erection chambers of the penis which can help to have a permanent erection which can also be easily concealed within the clothes.
Advancement in medical and surgical technology has helped to lower the risks of complications and failure of the devices remarkably. This procedure is even covered by majority of insurance providers all over the world.
There are three main types of Penile Implants.
1.Three-piece inflatable pump
The three-piece Inflatable pump consists of two cylinders in the penis, an inflatable pump in the testicle and a fluid filled container in the abdomen. All these components are connected to special tubing. Squeezing the pump in the testicle causes fluid to moves from the container into the cylinders, creating an erection. When the release valve is squeezed, fluid moves out of the cylinder and back into the container, making the penis flaccid. When inflated, a three-piece Inflatable pump planting works and feels like a natural erection. The Implant also feels natural and comfortable.
Placement – During placement of the device, the patient usually is under general or spinal anaesthesia. A skilled urologist trained in prosthetic urology performs the simple procedure.
Using a small incision, the urologist positions the inflatable cylinders in the corpora cavernosa. A small pump is positioned in the scrotum and a reservoir is placed inside the abdominal wall. The reservoir is filled with saline solution. The device is completely hidden and unobtrusive.
Recovery time – It commonly takes about four to six weeks to regain sexual function.
How it works. Gently squeeze the concealed pump in your scrotum several times. This moves the saline solution from the reservoir into the cylinders. As the cylinders fill, the penis becomes erect and firm.
To end the erection, simply press a “deflation site” on the pump. Deflating the cylinders transfers the fluid back to the reservoir and the penis becomes flaccid.
2.Two-piece Inflatable
This type of implant is similar to the three-piece Inflatable device and works the same way. However, instead of the container filled with a different fluid, the liquid is kept inside the pump in the testicle. However, the two pieces Inflatable device is not hard as a three-piece implant.
Placement- During the procedure, the patient usually is under either local or general anaesthesia.
A skilled urologist trained in prosthetic urology performs the procedure which usually takes 45 minutes or less. Using a small incision, the urologist positions the inflatable cylinders in the corpora cavernosa. A small pump is positioned in the scrotum. The device is completely concealed and unobtrusive.
The pump inflates the cylinders by transferring fluid within the system. A simple deflation technique then transfers fluid to return the penis to a flaccid state.
Recovery time- It takes about four to six weeks to regain sexual function.
How it works – Gently squeeze and release the concealed pump in your scrotum several times. This transfers the fluid in each cylinder to the cylinder shaft. Your penis will then become erect.
3.Semi-rigidor Malleable Rods
In this type of penile implant, the surgeon imposes two flexible rods in the penis. Regardless of the circumstances, transplants do not change in size or hardness and maintain a semi-hard state. However, they can be easily set to different positions. The implants are usually bent downwards, but it can be straightened in the upper position when necessary for sexual intercourse.
Placement- The surgeon gently places two bendable, “positionable” rods into the penis. The rods have an outer coating of silicone and inner stainless steel core or interlocking plastic joints. These enable the man to place the penis in either the erect or flaccid position.
Recovery time– It takes about four to six weeks to regain sexual function.
How it works -For intercourse, the man places his penis in the erect position. As he does so, the rods inside his penis bend. To conceal the penile implant, the man bends his penis down. He can bend the device in more than one place to create the desired erection.
What happens following a Penile Implant surgery?
The patient is transferred from the operating room to the recovery room. Vital signs of the patient are monitored constantly to ensure that there is no immediate risk of complications from the surgery. Painkillers are usually prescribed for a short while to cope with any pain. In case of a simple surgical procedure, the patient is discharged from the hospital the same day. In other cases, the patient is normally transferred to another room for a few days.
What is sex using an implant like?
When the implant is inflated or moved into position, it has a similar feel to a man’s regular erection, both in girth and stiffness.
Some men report that the implant does not make their erection last as long as usual, although some newer implants have addressed this issue.
Another difference is that implants do not affect the head of the penis, meaning that it does not become hard.
Penile implants do not affect the way that sexual intercourse feels; neither do they affect a man’s ability to have an orgasm or ejaculate.
Penile Implant Surgery Cost
Penile implant surgery can cost between USD 3000 to USD 15000 depending on the type of implants. The malleable semi rigid implants are cheaper while the Inflatable implants from AMS or Coloplast are expensive. 2 Piece AMS implant would cost around USD 10000 while the 3 Piece would be closer to USD 15000. The cost also varies from the hospital that you choose and also the surgeon who is going to do the surgery.
Common Questions you might Have
How and what device is used for Penile Implant Surgery?
Through a 3-4 cms incision at the level of penis and scrotum, small openings are made on both carpora cavernosa and after taking accurate measurement (length) implants are selected and placed along with the pump.
How long does the penile implant procedure take to complete?
This procedure takes approximately 1.30 hours to 2 hours.
Will I have normal feelings after implant?
The penile implant does not interfere with the natural feelings. This device has nothing to do with the ejaculation or orgasm.
Does a penis with implant look and feel like a natural erection?
Penile Implant Surgery is not noticeable when your penis is flaccid. Your partner would not be able to make out that you have had an implant in your penis unless you tell her.
What if I get an infection after the procedure?
If it is a superficial infection then can be treated with antibiotics. And if the implant gets infected then we have to replace it with the new one.
What are the possible risks/complications of penile implant?
Post-operative complications of penile implant surgery are not common. There are some risks associated with the implant procedure including pain, anaesthesia, and repeat surgery due to infections or mechanical failure of the device.
Which specialist/surgeon will do the surgery?
A Urologist who is specialized in Genito-Urinary Reconstructive Surgery is the right choice for penile prosthesis surgery. A routine urologist may not be efficient to perform this procedure.
What do I need to prepare for the penile implant procedure?
A patient needs to prepare certain essentials before heading towards the penile implant surgery.
Few are listed below:
i. Get your medical history reviewed by the Urologist who is specialised in Genito-Urinary Reconstructive surgery.
ii. Do a physical examination: Physical evaluation is must prior the surgery to avoid any hassles while surgery,
iii. If diabetic, sugar needs to be controlled.
iv. Any skin infection should be treated prior surgery.
v. Penile sensation should be intact.
What are the post- operative precautions to be taken?
• No sexual activity for 6 weeks after the surgery.
• No strenuous work for 4 weeks after the surgery.
What is the recovery time after the procedure?
A patient following penile implant surgery usually recovers within 2 days means he can resume his normal routine after 2 days of the surgery. You can begin enjoying sex once again after 6 weeks.
What is the cost of the surgery?
Surgery cost entirely depends on upon the cost of the implant chosen by the patient.
Do the insurance companies cover penile implant procedure?
Usually, it is advisable to coordinate with your respective insurance company in view of the same. What is
What is the success rate of the procedure?
About 90%-95% of inflatable implants produce erections suitable for intercourse.
More than 90% of men are moderately or completely satisfied with the results.
How safe is Penile Implants surgery?
Penile surgery is meant to cut the ligaments that suspend the front end of the penile shaft. This results in an extended penile shaft after the surgery, giving an illusion of a longer penis size. A major procedure of post-surgical recovery is to extend the penis for some time to prevent the ligaments from healing and becoming shorter than their original size. Without constant stretching, the results may not be successful.
To increase the girth of the penis, the surgeon mostly relies on fat cells that are sourced from the patient’s abdomen. These are processed and injected into the patient’s penis. This fat transfer procedure simply helps to increase the width of the penis overall. The girth can effectively be increased anywhere between 1 and 3 inches. A potential risk in this procedure is that the transferred fat cells feel abnormally soft. Also, the additional girth may also decrease if the fat cells are re-absorbed into the body eventually. At times, this re-absorption process is not uniform all over the penis and results in a lumpy and an abnormal-looking penis.
To combat this complication, an advanced technique called the ‘dermal transfer’ method is useful. This requires transferring thin strips of skin along with fat cells into the penis. The skin strips prevent the fat from being re-absorbed into the body. The grafting procedure involved with this technique makes this a slightly more complex surgical procedure. There is also a greater risk of penis disfigurement.
Penis lengthening procedures are significantly different than those used to increase the girth of the penis. The corpora cavernosa is the tube of tissue inside the penile shaft which fills with blood and results in an erection. At times, a significant portion of this tube may be concealed within the body. The penis lengthening procedure relies on releasing the concealed portion of tissue to give the entire length to the penis. The concealed portion of the corpora cavernosa is cut to release the stricture. This adds noticeable length to the penile shaft.
The fact that the surgery removes the supportive ligament of the penile shaft from the pubic bone may result in a ‘soft’ base of the penis. This may result it in slipping and shifting while having sexual intercourse. Advances in surgical techniques have remedied this problem to a large extent today.
