Gynecomastia Types
Gynecomastia can be categorized into Glandular gynecomastia, fatty gynecomastia, and loose skin in the breast region.
Glandular Gynecomastia:
The result of a surplus of estrogen in the male body is glandular gynecomastia. More and more men are becoming estrogen-dominant due to environmental factors like plastic, pesticide-laden foods, and drugs. The glandular breast tissue may become swollen when this occurs.
Fatty Gynecomastia:
When excess fat cells begin to accumulate in the breast region, excess weight gain may cause fatty gynecomastia, aka pseudo-gynecomastia. Gynecomastia and pseudo-Gynecomastia appear the same on the surface. But there is a central difference: the swollen portion of your chest.
Excessive Skin Gynecomastia:
Your skin will become stretched if you lose a large amount of weight without effectively toning your chest. The size of the breasts can not change, even though you don’t have as much fat under your breast tissue. In these cases, cosmetic surgery is the safest choice, focusing on removing the excess skin by excision.

Liposuction Technique:
Liposuction methods alone can be used in situations where gynecomastia is mainly the product of excess fatty tissue. This includes a cannula, a thin hollow tube, to be inserted through multiple small incisions. In a controlled motion, the cannula is rotated back and forth to loosen the excess fat, which is then extracted from the body by vacuum suction.

Excision Technique
Excision procedures are advised if it is appropriate to remove glandular breast tissue or excess skin to correct gynecomastia. If the areola is lowered or the nipple is repositioned to a more natural male contour, excision is also required. Depending on the particular circumstances and the surgical preference, incision patterns differ.
Combination of liposuction & excision techniques
Sometimes gynecomastia is treated with both liposuction and excision.
Gynecomastia Grades
Grade 1 – Very Mild – Small enlargement without skin excess
Usually, below the areola, the breast has a localized region of tissue. Sometimes referred to as “puffy nipple.” In general, the consistency of the skin is very strong. Just excess glandular tissue is found in most cases.
Grade 2- Mild to High – Moderate enlargement without skin excess
Moderate breast enlargement with indistinct borders from the chest region that goes beyond the areola. It can consist of a mixture of fatty tissue, glandular tissue or both.
Grade 3 – High and Visible – Moderate enlargement with minor skin excess
The same as moderate breast enlargement, but skin redundancy is also present. May consist of fatty tissue, glandular tissue or both.
Grade 4 – Severe and Feminized – Marked enlargement with excess skin, mimicking female breast ptosis
With major enlargement and skin laxity, the breasts appear feminine. It could look maternal. When the condition of the skin worsens, the breasts appear worse than they did when the skin was tight, most commonly seen in aged men.
